What Are the Best Piercing Jewellery Materials?

The material your piercing jewellery is made from determines whether your fistula heals cleanly or not at all. Implant-grade titanium is the gold standard for initial piercings. This guide covers every approved material, explains what ASTM F136 means, why surgical steel is not the same as implant-grade steel and what materials you should never wear in a fresh or healing piercing.

Why Material Matters

What Are the Best Piercing Jewellery Materials?

The jewellery in a healing piercing is not decorative: it is a medical device that the body must accept and heal around. A material that contains nickel or other reactive compounds triggers chronic low-grade inflammation in the tissue trying to form the fistula. The body cannot build a clean, smooth healing channel around a chronically irritated object. The result is thicker scar tissue, prolonged healing or complete failure to heal. In the worst cases the body treats the jewellery as a persistent foreign irritant and rejects it entirely.

The Association of Professional Piercers (APP) publishes minimum material standards for initial piercing jewellery. These standards require materials that are biocompatible (non-reactive with body tissue), non-porous (no microscopic surface irregularities where bacteria can harbour), able to withstand autoclave sterilisation and polished to a mirror finish. Any jewellery sold for piercing that cannot be verified against these standards should not be used in a fresh or healing piercing regardless of price.

Mill certificates are the documentary proof that a material meets a specific ASTM or ISO standard. A reputable studio can produce mill certificates for the jewellery they use. If a supplier or studio cannot provide mill certificates for their initial piercing jewellery, this is a meaningful warning sign. The certificate is not bureaucracy: it is the only verifiable proof that the alloy inside the jewellery matches the specification printed on the label.


Approved Materials

Which Materials Are Safe for Piercing Jewellery?

Implant-grade titanium The gold standard ASTM F136 (Ti-6Al-4V ELI, Grade 23) is the specification used by the NHS for bone pins and joint replacements. It is 100% nickel-free, hypoallergenic, lightweight (50% lighter than steel), non-corrosive and fully biocompatible. It can be anodised to produce colours by altering the oxide layer on the surface: no dyes or plating are involved so the surface cannot chip off inside a healing wound. The first choice for all initial piercings at any responsible studio.

Implant-grade surgical steel Acceptable with caveats ASTM F138 (316LVM) is the steel standard approved for initial piercings. It is not the same as generic "surgical steel": the term surgical steel describes grades used for surgical instruments which do not necessarily meet implant biocompatibility standards. Implant-grade steel contains trace nickel (typically 10 to 14%) which is bound within the alloy. Most people tolerate it well. Anyone with a documented nickel sensitivity should use titanium instead.

Niobium Titanium-like properties Niobium (999 niobium, at least 99.9% pure) is biocompatible, nickel-free and can be anodised like titanium. It does not hold an official implant-grade designation but has an excellent long-term safety record in the piercing community. It is heavier and softer than titanium, which makes it useful where flexibility is needed (for example, bending rings to anatomical shapes). Less widely available than titanium and more expensive to produce.

Surface Finish

Why Mirror Finish Matters as Much as Material

Even the correct material will cause problems in a healing piercing if the surface finish is poor. The APP requires that body jewellery for initial piercings be polished to a smooth mirror finish with no nicks, scratches, burrs or tool marks. New tissue cells growing to line the fistula channel during healing attach to the jewellery surface. If the surface has microscopic irregularities, the cells grow into them. When the jewellery moves, these attachment points tear. This cycle of tearing and re-healing creates excessive scar tissue and prolongs healing indefinitely.

Mirror-polished implant-grade titanium with internally threaded or threadless closures is the combination that professional studios use for initial piercings. Externally threaded jewellery has threads on the post that the ball screws onto: those threads pass through the fresh piercing channel during insertion and tear the delicate new tissue. Internally threaded or threadless jewellery has a smooth external surface: the closure mechanism is inside the post rather than on it.

The practical rule: if you cannot verify the material specification with a mill certificate and the studio cannot confirm the jewellery is internally threaded or threadless with a mirror-polished finish, ask for what they can confirm or book with a studio that can.
