Gauge measures the thickness of the post or wire that passes through a piercing. It measures nothing else: not the length of a barbell, not the diameter of a ring. Gauge = post thickness only. In that system, the gauge number referred to how many times wire was drawn through progressively smaller dies: more draws produced thinner wire and a higher gauge number. Body jewellery adopted the same system, which is why the numbers appear backwards compared to what most people expect.
The practical rule to memorise: higher gauge number means thinner post. A 20G nose stud (0.8mm) is considerably thinner than a 14G navel bar (1.6mm). The numbers do not proceed uniformly: they jump in even increments (20G, 18G, 16G, 14G, 12G, 10G, 8G, 6G, 4G, 2G, 0G, 00G) before switching to fractional inch measurements for stretched lobe sizes. Each step represents a meaningful increase in post thickness.
Why does gauge matter? Because the fistula heals to the gauge of the jewellery inside it. Insert jewellery that is thinner than your piercing was done at and the channel narrows. Insert jewellery that is too thick and you are trying to force a wider post through a narrower channel, which causes pain and tissue damage. Always match new jewellery to the gauge of your existing piercing. If you are unsure what gauge you were pierced at, ask your piercer rather than guessing.

What Happens If You Use the Wrong Gauge?
Using jewellery that is thinner than your piercing was done at causes the fistula to gradually narrow to fit the thinner post. Over months this can result in a narrowed channel that resists accepting the original gauge jewellery in future. If the narrowed channel is then stretched back to the original gauge without proper preparation, tearing and irritation result.
The cheese-wire effect is what happens when very thin jewellery is worn in a placement that experiences regular movement or pressure: the thin post acts like a wire being worked through soft tissue, gradually cutting through the fistula wall. This is the primary reason that 20G jewellery should only be used in the most stable, low-movement placements (nostril and some ear lobes). Using 20G jewellery in a tongue, navel or lip piercing risks the post slowly migrating through the tissue.
When you buy new jewellery for a healed piercing, confirm the gauge with your piercer before purchasing online. The gauge may be printed inside the packaging in small text.
