Your Complete Guide to HCH Piercings
An HCH piercing passes horizontally through the clitoral hood side to side. It is the horizontal alternative to the more popular VCH and is primarily chosen for its aesthetic appeal and the lateral stimulation it provides. This guide covers how it differs from the VCH and triangle, the anatomy requirement, pain levels, the 4 to 8 week healing timeline and aftercare.Your Complete Guide to HCH Piercings
An HCH piercing passes horizontally through the clitoral hood from one side to the other. A captive bead ring or circular barbell sits through this horizontal channel with the ring body or the two ball ends visible on either side of the hood. HCH stands for Horizontal Clitoral Hood: the "horizontal" distinguishes it from the VCH (Vertical Clitoral Hood), which passes from top to bottom through the peak of the hood.
The HCH is primarily chosen for aesthetic reasons. Unlike the VCH whose barbell rests against the clitoris from above to create direct stimulation, the HCH jewellery sits horizontally across the hood and does not make direct contact with the clitoral glans in the same way. Indirect stimulation from the ring moving against the hood tissue is possible but the HCH is generally considered a more aesthetically focused piercing than a functionally stimulating one. People who want a visible horizontal ring through the hood with a distinctive look often prefer the HCH.
The HCH is anatomy-dependent. The clitoral hood must be pronounced enough, convex and symmetrical enough to support a horizontal ring sitting flat across it. A hood that is flat, narrow or does not stand away from the outer labia will not allow the ring to lie flat when the legs are closed, which leads to poor healing and discomfort. The VCH works for more anatomies than the HCH: if you have been assessed as unsuitable for a VCH it does not automatically mean you are suitable for an HCH. The reverse is also true. In the UK, genital piercings require clients to be 18 or over with valid photo ID.
How the HCH Compares to Other Hood Piercings
Does an HCH Piercing Hurt?
Most people rate HCH piercing pain at 4 out of 10. The clitoral hood is highly vascular sensitive tissue but it is thin and the needle passes through quickly. Many people report that the anticipation is more difficult than the actual procedure. The sharp pinch is brief and the soreness afterwards is mild compared to most genital piercings. Some light bleeding for the first few days after the procedure is normal.
Initial healing takes 4 to 8 weeks, one of the faster healing timelines for a genital piercing. The highly vascular hood tissue means excellent blood supply reaches the site and healing progresses rapidly. The outer surface of the piercing may appear healed well before the internal fistula has fully matured: do not change jewellery or resume sexual activity until your piercer confirms full healing.
| Stage | Timeframe | What to Expect |
|---|---|---|
| Inflammatory | Days 1–7 | Mild tenderness; possible light bleeding; liner in underwear; no sexual activity |
| Healing | Weeks 1–4 | Sensitivity reduces quickly; clean twice daily; avoid friction from clothing |
| Healed | Weeks 4–8 | Confirm with piercer; jewellery can be changed at professional appointment; resume sexual activity with protection |
What Jewellery Is Used in an HCH Piercing?
A captive bead ring is the most commonly used jewellery for an HCH piercing. The ring passes through the horizontal channel with the bead visible at the front of the hood. A circular barbell is an equally appropriate alternative. Ring-style jewellery suits the HCH placement better than straight barbells: the ring body curves to accommodate the lateral geometry of the horizontal channel and creates a more comfortable fit against the hood tissue during healing and long-term wear.
| Style | Best For | Size Guide | Notes |
|---|---|---|---|
| Captive bead ring (initial) | Fresh and healing | 12G or 14G; diameter to anatomy | Standard starting jewellery; ring must lie flat against the hood when closed |
| Circular barbell (initial or healed) | Fresh or healed | 12G or 14G; diameter to anatomy | Horseshoe shape; slightly adjustable diameter; good alternative to captive ring |
Implant-grade titanium only during healing. The ring diameter must be sized so the ring lies flat when the legs are fully closed: a ring that does not lie flat creates constant movement and pressure against the hood tissue that prevents healing and causes chronic irritation. Sizing is done by your piercer at consultation: do not self-select ring diameter for this placement.

How to Care for an HCH Piercing
Clean the piercing with sterile saline twice daily. Submerge the hood area in a clean container of warm sterile saline for 2 to 3 minutes. Alternatively spray saline directly at both ends of the ring. Pat dry carefully with a clean paper towel after every soak and after showering. Keeping the area dry between cleans is important: moisture trapped around the ring can slow healing at this placement.
Wear clean breathable cotton underwear throughout healing. Change daily. Avoid tight underwear or clothing that creates direct friction or pressure against the piercing. Lining underwear with a clean pad for the first week manages any light spotting and keeps the area cleaner during the early healing phase.
Avoid sexual activity during the full healing period. Barrier protection is required for the first 6 months after confirmed healing when resuming sexual activity. Rinse the piercing with saline after any sexual activity throughout the healing period and for at least 3 months after confirmed healing. Avoid swimming pools, baths and hot tubs throughout healing. Monitor that the ring continues to lie flat against the hood: if the ring appears to be sitting at an angle or one bead appears to be pressing into the skin rather than resting on the surface, visit your piercer for assessment.
