What Is an Ampallang Piercing?

An ampallang piercing passes horizontally through the glans of the penis from one side to the other. A straight barbell sits through this horizontal channel with a ball end visible on each side of the penis head. The name derives from the Dayak people of Borneo where the palang (crossbar) was a traditional rite of passage for young men. In contemporary piercing practice the term ampallang refers specifically to the horizontal glans piercing, in contrast to the apadravya which is the vertical equivalent.

The ampallang can be placed in two distinct ways depending on the anatomy and preferences discussed at consultation. The American ampallang is placed above the urethra, passing through only the spongy tissue of the upper glans and typically set back near the coronal ridge. The European ampallang is placed lower and further forward, passing through or very near the urethra. The European placement passes through less tissue due to its lower position where the glans is narrower and tends to heal faster for this reason. Both styles create the same visual effect with different healing profiles.

The ampallang is among the most technically demanding of all genital piercings. The horizontal channel must pass through the full width of the glans with perfect symmetry and a level horizontal plane: any deviation creates a crooked placement that will never sit correctly and may never heal. Only a piercer with specific documented expertise in trans-glans horizontal work should attempt this piercing. In the UK, genital piercings require clients to be 18 or over with valid photo ID.

Pain and Healing

Does an Ampallang Piercing Hurt?

The ampallang is widely considered the most painful of the commonly performed male genital piercings, typically rated at 9 out of 10. The glans contains one of the highest concentrations of nerve endings in the male body and the horizontal channel passes through more tissue than the apadravya due to the greater width of the glans in the horizontal plane. Bleeding during and after the procedure is expected and significant: your piercer will apply a compression wrap immediately after. Lining underwear with pads for the first 5 to 7 days is essential.

Healing takes 6 to 12 months, longer than the apadravya. The American placement (non-urethral) heals more slowly than the European placement (urethral) because it does not benefit from the flushing action of urine. The barbell must be long enough to accommodate the fully erect measurement: the glans widens significantly during erection and a barbell that fits flaccid may create painful compression when erect if not sized correctly for the erect state.

What Jewellery Is Used in an Ampallang Piercing?

A straight barbell is the only appropriate jewellery for an ampallang piercing. The horizontal bar passes through the glans with a ball end visible on each side of the penis head. The barbell length must accommodate the fully erect measurement of the glans width: the glans widens substantially during erection and under-sizing the barbell creates tissue compression and healing failure. A minimum gauge of 10G or 12G is recommended: thinner jewellery in a full-width glans piercing creates tissue tear risk particularly during erection.

Aftercare

How to Care for an Ampallang Piercing

Submerge the glans in a clean glass of sterile saline for 3 to 5 minutes twice daily. This allows solution to reach both horizontal exit points and the full channel simultaneously. Alternatively spray saline generously at both ball ends. Pat dry carefully with a clean paper towel after each soak. Pad underwear with clean liners for the first 5 to 7 days: the highly vascular glans tissue bleeds more freely than most other piercing sites and the initial bleeding period is longer for an ampallang than for most piercings.

Avoid all sexual activity during the full healing period. Any arousal during the healing period causes the glans to engorge, pulling against the healing channel and slowing progress. This is unavoidable during sleep: a barbell sized correctly to the erect measurement protects against compression during nocturnal erections. Avoid pools, baths and natural bodies of water throughout healing.

When sexual activity resumes after confirmed healing, use barrier protection throughout the first 6 months. Ensure condoms fit correctly over the barbell ends: the horizontal ball ends sit at the widest point of the glans and a standard condom must be rolled down carefully to accommodate them without creating tears. Drink 2 litres of water daily throughout healing to maintain urethral health (for European placements) and overall tissue healing quality.
