Yes, all piercings hurt to some degree. The difference between a 2 out of 10 earlobe and a 9 out of 10 ampallang comes down to tissue type, nerve density and cartilage thickness at the placement site. Every piercing involves a needle passing through living tissue. Pain varies because tissue varies. The three main anatomical factors that determine how much a specific placement hurts are nerve density at the site, tissue type (soft flesh vs cartilage vs erectile tissue) and the thickness of material the needle must pass through. A thin earlobe with few nerve endings passes a needle through in a fraction of a second. A dense cartilage fold with high nerve concentration resists the needle and creates more sensation over a longer pass.

Individual pain tolerance is the other major variable. Two people can receive identical piercings from the same piercer on the same day and have entirely different experiences. Factors that affect personal pain tolerance on the day include sleep quality the night before, whether you have eaten beforehand, hydration level, anxiety and tension in the body and whether this is your first piercing or your fiftieth. Regular piercers often report that the anticipation of pain is harder than the actual needle for many clients: the procedure itself lasts one to three seconds for most placements.

How to Prepare

How to Make a Piercing as Comfortable as Possible

The single most impactful preparation step is eating a full meal beforehand. Arriving in a fasted state drops blood sugar and dramatically increases the chance of feeling faint or lightheaded during or after the procedure. A proper meal 1 to 2 hours before your appointment is the most universally recommended preparation step across all piercing types.

Sleep well the night before. Fatigue reduces pain tolerance measurably. Drink enough water: well-hydrated skin pierces more cleanly and recovers faster than dry, dehydrated tissue. Avoid alcohol for at least 24 hours before your appointment: alcohol thins blood, increases bleeding and lowers pain tolerance. Avoid aspirin and ibuprofen for the same reason. If you need pain relief after the piercing, paracetamol is the correct choice for the first 24 hours.

Control anxiety by planning what to focus on during the needle pass. Most placements are complete in one to three seconds. Having a mental anchor (a slow exhale, a focus point in the room) and telling your piercer to let you know the moment before they pierce helps many people. Arriving early enough to settle, reading the aftercare information and asking any outstanding questions before the procedure all reduce the anxiety that amplifies pain perception. Choose an experienced piercer for any placement: a confident, fast, well-placed needle always hurts less than a slow, hesitant or poorly positioned one.
