A Complete Guide to Frowny Piercings
The frowny piercing passes through the lower lip frenulum inside the mouth and is hidden unless the lower lip is pulled down. This guide covers why it is rarer and harder to get than the smiley, the anatomy check required first, low pain levels, 4 to 8 week healing and rejection risk.A Complete Guide to Frowny Piercings
A frowny piercing passes through the labial frenulum of the lower lip: the thin strip of tissue that connects the inside of the lower lip to the lower gum. It is the lower lip counterpart to the smiley piercing, which passes through the equivalent frenulum of the upper lip. The frowny is also called an anti-smiley piercing or a lower lip frenulum piercing. The jewellery sits entirely inside the mouth and is completely hidden when the mouth is closed or at rest. It becomes visible only when the lower lip is pulled downward.
The frowny is considerably rarer than the smiley for a direct anatomical reason. The lower labial frenulum is typically thinner and shorter than the upper labial frenulum. Many people do not have enough lower frenulum tissue to safely hold jewellery: a too-thin or too-short frenulum cannot provide stable anchorage and would tear rather than heal around the jewellery. A higher proportion of people are turned away from the frowny than from the smiley for this reason.
The concealment profile of the frowny is more absolute than the smiley. Where the smiley reveals naturally when you smile or open your mouth, the frowny stays hidden during all normal facial expressions including smiling. You must deliberately pull the lower lip down to expose it, which means the frowny is entirely on your own terms. This makes it one of the most controllably hidden piercings available.

Does a Frowny Piercing Hurt?
Most people rate frowny piercing pain at 2 to 3 out of 10. The lower labial frenulum has very few nerve endings and the needle passes through the thin tissue in under a second. The sensation is typically a brief pinch. Minor swelling below the lower lip in the first 2 to 3 days is normal and resolves quickly. Most recipients find the frowny one of the most tolerable piercings they have experienced.
Healing is fast for the same reasons as the smiley: the mouth has excellent blood supply and oral tissue regenerates rapidly. Most frowny piercings complete healing within 4 to 8 weeks with consistent aftercare. However the frowny shares the smiley's most significant long-term risk: the thin frenulum tissue is prone to rejection and migration over time regardless of how well aftercare is followed.
| Stage | Timeframe | What to Expect |
|---|---|---|
| Initial | Days 1–5 | Mild tenderness; slight awareness of jewellery when eating; resolves quickly |
| Downsize window | Around week 4 | Replace initial longer bar with shorter fitting bar to reduce lower tooth and gum contact |
| Healing | Weeks 4–6 | Increasingly comfortable; easy to forget jewellery is there; continue aftercare to piercer confirmation |
| Full healing | Weeks 6–8 | Confirmed by piercer; can switch to alternative jewellery; monitor for migration throughout its life |
What Jewellery Is Used in a Frowny Piercing?
A captive bead ring is the most common starting jewellery for a frowny piercing, following the same curve of the lower frenulum tissue as the smiley's captive bead ring follows the upper frenulum. A circular barbell (horseshoe) is an equally common alternative with two ball ends visible when the lower lip is pulled down. The ring format follows the natural tissue curve and is gentler on the lower frenulum than a straight bar under the tongue's resting pressure.
| Style | Best For | Size Guide | Notes |
|---|---|---|---|
| Captive bead ring | Fresh and healed | 16G, 6–8mm inner diameter | Most common starting jewellery; ring follows lower frenulum curve naturally |
| Circular barbell (horseshoe) | Fresh and healed | 16G, 6–8mm inner diameter | Two ball ends visible on reveal; horseshoe format similar to a septum ring |
| Seamless ring | Healed piercings | 16G, 6–8mm inner diameter | Minimal and elegant; no visible closure mechanism; understated reveal |
Implant-grade titanium only. Never use sterling silver in any oral piercing. Choose the smallest appropriate diameter to minimise weight and leverage on the lower frenulum. Excess weight is the primary controllable factor in accelerating rejection in thin frenulum tissue. Downsize diameter promptly once swelling resolves to remove unnecessary length that contacts the lower teeth and gum.

How to Care for a Frowny Piercing
Because the frowny is entirely inside the mouth, aftercare focuses on oral hygiene rather than external cleaning. Rinse with alcohol-free mouthwash or sterile saline after every meal and drink throughout the healing period. Maintain thorough general oral hygiene throughout: brush teeth twice daily being careful not to hit the jewellery with the toothbrush head. Good oral hygiene handles much of the bacterial management that external piercings require dedicated saline spray to address.
Do not repeatedly pull the lower lip down to show the piercing during healing. Every deliberate stretch of the frenulum applies tension to the healing wound. Avoid smoking if at all possible: nicotine slows the body's healing response and dries oral tissue. Avoid alcohol for at least four weeks. Avoid spicy and acidic foods for the first two weeks.
Monitor the jewellery position throughout the life of the piercing, not just during healing. The ring should appear stable at a consistent depth within the lower frenulum. If the ring appears to be riding closer to the tissue's front edge over time: tissue thinning above the ring is a sign that migration is beginning. Visit your piercer promptly if this is observed: removing the jewellery before the frenulum tears prevents permanent scarring of this tissue.
